    Description

    Hometown Care is a single site, family owned agency based out of Summit County, located right on the Cuyahoga River between Broad Blvd and Portage Trail exits off Route 8. We are a growing agency looking for a PRN LPN to assist our RN Case Managers with routine patient visits in our territory.

    We seek an experienced home health nurse to pick up visits on a PRN/part-time basis.

    Responsibilities

    1. Implements the nursing care plan for each client as established.

    2. Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.

    3. Initiates preventive and rehabilitative nursing procedures as appropriate for the client's care and safety.

    4. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the client's physical or emotional condition.

    5. Teaches and counsels the client and family/significant others regarding the nursing care needs and other related problems of the client at home and assure their involvement in implementing the plan of care.

    6. Prepares clinical and progress notes.

    7. Evaluates with registered nurse the effectiveness of the LPN's nursing service to the client and family under the guidance of the registered nurse.

    8. Maintains accurate and complete records of observations, treatments, and care of client.

    9. Participates in medical record audit as assigned.

    10. Attends staff meetings and in-services as scheduled.

    11. Takes on-call duty, nights, weekends and holidays as assigned.

    12. Assist the physician and RN in performing specialized procedures.

    13. Prepares equipment and materials for treatments.

    14. Observes aseptic technique as required.

    15. Assists the client in learning appropriate self-care techniques.

    16. Submit any changes in schedule to Director of Clinical Services on a daily basis.

    17. Participate in client care conferences to discuss the need for involvement of other members of the health team such as physical therapist or speech language pathologist.

    18. Accept, write, and execute orders; orders are cosigned by RN.

    19. Administer medications and treatments as ordered.

    Provide appropriate information regarding client to other agencies/individuals involved in client's care as needed.

    Qualifications

    Current nursing license

    Home Health experience

    Driver's license
