    Job Summary:
    Provides direct clinical social work services including counseling, discharge planning, and case management for hospital patients and families. Works collaboratively with the interdisciplinary healthcare team to support patient care across the continuum.


    Responsibilities:
    Conduct psychosocial assessments
    Develop and implement discharge plans
    Provide counseling and emotional support to patients and families
    Coordinate with community resources for follow-up care
    Document in medical records and electronic systems (Cerner, MIDAS+)
    Collaborate with physicians, nursing, and care team
    Monitor patient progress and address barriers to discharge
    Support quality improvement and care coordination initiatives

    Requirements:
    Master of Social Work (MSW) from CSWE-accredited program
    ASW or higher license required (LCSW preferred)
    Acute care hospital experience required
    Strong psychosocial assessment and clinical decision-making skills
    Experience with discharge planning and case management
    Strong communication and documentation skills
    Ability to work with diverse patient populations (neonatal to geriatric)
