Job summary: Promote safe use of antimicrobial agents across University Hospital.
Responsibilities
- Leads assessment prioritization planning implementation of antimicrobial therapy management initiatives.
- Designs patient-specific antimicrobial dosing regimens protocols approved by Pharmacy & Therapeutics Committee.
- Screens monitors patients on antimicrobial therapy to ensure appropriate agent selection dosing duration evaluates for IV-to-oral conversion opportunities adjusts doses for renal hepatic impairment performs pharmacokinetic assessments to optimize therapy.
