    Description
    Enters into tEnters into the private homes of PACE enrollees or the facilities or homes of patients.

    Follows scheduled transportation for PACE participants or patients.

    Handles vehicle maintenance issues as directed by coordinator or supervisor.

    Assures that all necessary vehicle records are maintained according to policy.

    For PACE Only: Participates as a member of the interdisciplinary team.

    Communicates participant changes to coordinator or supervisor.

    For PACE Only: Participates in the development of participant plan of care.

    Completes all documentation in the medical record as required.

    For PACE Only: Must meet a standardized set of competencies for the specific position established by Centra Pace and approved by CMS before working independently.

    May perform other duties as assigned or requested and job specification can be modified or updated at any time.

    Preferred Education: High School diploma or GED Preferred Experience: For PACE drivers only: One year's experience working with a frail and elderly population exhibiting the skills needed to assist geriatric patients with transportation needs. Required Certification & Licensures: Valid Virginia driver's license. Needs to be completed prior to the end of the orientation period and before being released to drive solo: CPR First Aid Transfer, Stair, and Gait training Wheelchair training Fall prevention training Bed Mobility Emergency Vehicle Operators Course (EVOC). For ground transport drivers only: Needs to be completed before the orientation period ends and before being released to drive solo. For PACE drivers only: Needs to be completed within the first six (6) months of hire.
