- Current RN license for Pennsylvania
- Valid Driver's License and reliable vehicle
- Prior community-based experience in a Medicare-certified home health and/or hospice/palliative agency preferred.
- CPR Certification
- Work at least one night per week in Hospice
- Work at least one week per month in Home Health
- Work one weekend per month in Home Health or Hospice
- Company Car (Subaru) with gas card
- Flexible daytime schedule with rotating weekend on-call

Job DescriptionEklego is seeking an RN Case Manager for our client's Home Health and Hospice division. Eklego partners with choice Senior Care organizations committed to hiring qualified and compassionate staff, and Eklego partners with YOU, as a healthcare professional, to find a workplace that best fits your priorities
What will I do as an RN Case Manager? This is a direct-hire, full-time position, working in the field, on-call, and in the office. Standard hours are Monday, Wednesday, Friday 8a-6p with flexibility to pre-schedule an off day on Tuesday or Thursday. The Registered Nurse Case Manager works independently throughout Greensburg, PA, providing initial and ongoing assessment of client's skilled nursing needs. The RN Case Manager will also implement care plans and make referrals to other services that support the patient's care plan. You'll also be provided with all the supplies you'll need for documentation and treatment. Requires:
