- Admits patients to service, assesses the patient's and family's physical, psychosocial,environmental, safety and developmental needs. Develops an individualized plan of care based

- Maintains the dignity of the dying patient and supports the patient's and family's unique
- Provides holistic, family-centered care across the treatment settings to improve the quality of
- Knowledgeably discusses and initiates non-pharmacologic interventions as appropriate to
- Participates in the hospice program's quality assessment performance improvement program
- Meets productivity standards as established by the manager
- When in a facility, adheres to "Pathways Standards of Care and Practice in Facilities" or
- Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facility
- Maintains timely communication with patient, family, and facility staff regarding all aspects of
- Demonstrates awareness of and adherence to the safety, infection control and other policies of
- Maintains and updates level of knowledge and skills required to provide quality nursing care
- Creates, documents, and maintains the medication list. Evaluates medications for side effects
- Effectively instructs patient, family, and/or staff use of medications and reconciles medication
- Effectively communicates with and uses the Hospice prescription plans, formulary/preferred
- Adheres to all agency policies and procedures, including updates, as communicated in written,
- Promotes a culture of service when representing Pathways by at all times treating internal and
- Assists in development of Pathways protocols, procedures and policies as requested
- Demonstrates all skills of a Registered Nurse, including those skills required by the agency
- Performs other duties as assigned consistent with skills and training and the mission and goals
- California RN license. Hospice or Home Health experience preferred
- Graduate of an NLN accredited nursing school
- Must be highly motivated, self directed, flexible, and able to work well and participate in a
- Minimum of one-year experience as a professional nurse within the last three years or a
- California driver's license and automobile insurance with minimum Bodily Injury Liability
- Current CPR Certificate– American Heart Association "BLS Health Care Provider" requested

The posted compensation range of $ $78.30/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. POSITION SUMMARY: The Shared Services After Hours Visit RN is responsible for providing skilled nursing visits to patients and their families as requested by Intella Triage. Documents observations and interventions related to the patient's environment; physical, mental, emotional and social status. Coordinates care with other
members of the Interdisciplinary Group (IDG) or team, including the patient's physician.
