    Under the direction and supervision of the Nurse Manager, the LPN assists in the performance of selected acts, for coordinating and supplementing patient care services in accordance with the Florida Nurse Practice Act.

    Position Details

    • Compensation: $ /hr
    • Days/Hours: Nights (7pm-7am); 12 hr/shifts
    • PRN Eligibility: Maintain at least one (1) shift a week over a 5 of 6 week scheduling period; two (2) shifts a week maximum allowed. Willingness for weekend backups; may have to work holidays when needed.
    • Location: In-Patient Unit, Hospice of the Golden Isles
    Job Description:
    • Maintains established productivity level in the inpatient unit.
    • Assists in the documentation and maintenance of medical records.
    • Assists the Interdisciplinary Group (IDG) Registered Nurse (RN) in the development and provision of the patient's Plan of Care and recommends change/revision as necessary.
    • Assists the IDG Primary RN and/or physician in performing specialized duties, and procedures, and administers medications, as appropriate.
    • Utilizes an individualized, outcome driven approach in the delivery of patient care.
    • Communicates and updates the IDG Primary RN and physician in observed changes in patients' condition, and provides written summary as required.
    • Assists the patient/caregiver in learning appropriate self-care techniques.
    • Participates in, and assists with discharge planning.
    • Participates in IDG meetings and collaborates with team members in the identification, management, and/or resolution of the patient/family/caregiver needs/problems.
    • Maintains the highest standards of professional conduct in relation to information that is confidential in nature by maintaining patient confidentiality and privacy in all aspects of care. Shares information only when the recipient's right to know is clearly established according to HIPAA regulations.
    • Processes physician orders and takes physician verbal orders and reduces to writing for processing. Verbal medication orders verified by RN signature.
    • Maintains and orders all medical supplies for the inpatient unit.
    • Creates and maintains staffing schedules.
    • Facilitates communication with the attending physician, if applicable and the primary hospice physician upon death of patient, and/or transfer of patient to another facility/level of care.
    • Coordinates funeral home and ambulance services for transportation of patients.
    • Prepares equipment and materials for treatment adhering to aseptic techniques as required.
    • Assists in the inpatient unit disaster preparedness and is available for assigned duties throughout the disaster.
    • Engages in the limited administration of intravenous therapy (IV), only after appropriate IV Certification has been obtained.
    • Participates in Performance Improvement activities, as applicable.
    • Acts as a role model in confidentiality, professionalism, appropriate attire, and work attitudes.
    • Adheres to general inpatient and life safety standards.
    Education/Experience:
    • Current License as a Licensed Practical Nurse/ Licensed Vocational Nurse in the state of Georgia.
    • IV Certification in the state of Georgia.
    • Current CPR Certification.
