    Highlights

    Community Physician Network is seeking a fellowship trained Sleep Medicine physician to be based in Kokomo and eventually Westfield.


    • Opportunity to work with experienced, AASM accredited providers
    • No call & outpatient only practice
    • Monday through Friday; 8am - 5pm with a mix of virtual & in-person visits
    • Epic EMR
    • Competitive compensation and benefits package
    Licensure & Certification


    • Board Certified in Sleep Medicine
    • Eligible for medical licensure in Indiana
