    Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County.

    We are seeking a full-time Integrated Support Care Manager to support our Shirley, NY site.


    SUMMARY OF POSITION:
    Provides integrated care management services to patients using a harm reduction approach to address psychosocial barriers to care.

    Provides coordination of care, psychoeducation, casework counseling, linkage to community supports, referral follow up, and maintains communication with the patient and care teams.

    Attends ongoing training to address needs of patients including understanding of HIV, Substance Use, Hepatitis C, mental health, and chronic conditions.

    Performs job duties in compliance with systems and job specific standards.


    ESSENTIAL FUNCTIONS:

    • Completes Care Management Intakes/assessments.
    • Develops and monitors patient care plans.
    • Develops linkages and refers patients for additional support services.
    • Completes patient Recovery Support and or Crisis plans.
    • Communicates and works collaboratively with the patient and care team.
    • Provides Naloxone training to staff and patients.
    • Provides follow up on patient referrals.
    • Conducts/supports HIV testing and links patients to appropriate services and follow up.
    • Participates in case conferencing with care teams, inclusive of providers.
    • Maintains current case records and other documentation as required and prepares necessary reports on time.
    • Demonstrates knowledge of stages of change, substance use disorders, HIV, HCV, and other co-occurring conditions.
    • Completes on-going training to enhance skills and patient care.
    • Empowers, educates, and links patients to HIV, HCV, STI testing/treatment, PrEP, substance use, and other primary care services.
    • Conducts quality improvement projects and other duties as assigned

    EDUCATION/EXPERIENCE:
    Associate's degree in Health or Human Services related field with 2 years of related work experience. Bachelor's and CASAC preferred.
