Evergreen Village at Fort Wayne is seeking a Director of Nursing (DON) to join our team

Responsibilities:

• Conducts initial assessment of prospective residents, including off-site visits
• Performswellness assessments and develops individualized care plans based on the needsand preferences of each resident
• Delivershealth promotion services, offers disease prevention counseling, and providesself-care education to residents
• Engageswith residents and family members in a compassionate and impactful manner,creating a lasting and positive experience
• Fosterscommunication by regularly engaging with physicians, pertinent healthcareproviders, and family members
• Hires,trains, schedules, and leads the clinical team in providing exceptional careexperiences
• Coordinatesthe Community's infection control program while maintaining state regulatorycompliance
• Developand oversee the Community's medication management program
• Contributes toward quality assurance and performance improvement initiatives
• Complies with all rules and regulations regarding confidentiality and privacy of resident information

Qualifications:

• Maintainscurrent state nursing licensure, RN preferred
• Minimumof two (2) years nursing supervisory experience
• Priorsenior/supportive living experience strongly preferred
• Excellentwritten and verbal communication skills
• Mustpossess leadership and supervisory ability and the readiness to mentor,motivate, and inspire team members
• Demonstratesexceptional interpersonal skills and ability to serve with love, compassion, and dignity
