- Case Manager will work with low-income families who are in need for short-term financial assistance, case manager, landlord mediation, and other supportive services that result in maintaining stable, permanent housing.
- Perform assessments on all applicants for programs eligibility.
- Provide case management services to clients enrolled in the program.
- Collaborate with other community providers.
- Participate in case conferences.
- Accurately maintain paper files for clients enrolled in the program.
- Document all case notes, links to services, and treatments in HMIS.
- Maintain positive relationships with landlords and other providers.
- Prepare all reports that are required in a timely fashion.
- Relevant experience in an office environment, preferably in an administrative position
- Exceptional interpersonal and communication skills
- Excellent writing skills strong spelling, grammar, and punctuation
- Ability to work independently and excellent time management skills
- Intermediate MS Office skills (Word, Outlook, Excel)
- Bachelor s Degree in Psychology, Sociology, Social Work or related field; or three to five years of equivalent experience in social services field
