- Verify the patient's understanding of prescribed medications with patient counseling.
- Assure appropriateness of prescribed drug therapy through chart and patient screening.
- Provide drug information and health promotion/disease prevention information to patients and other healthcare professionals.
- Assist in the coordination and management of cardiology ambulatory care clinics: Provide guidance in scheduling, patient enrollment, space/staffing issues, coordination with dispensing processes, and other daily clinical activities as assigned.
- Develop and update evidence based, disease-state management protocols.
- Develop, implement, and maintain a pharmacist disease state management-credentialing program for each clinical program/service.
