    Job Summary
    We are looking for a Home Health Aide to join our team You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
    Responsibilities
    • Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
    • Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
    • Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
    • Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
    • Give medications and treatments
    • Engage patients in exercises or other activities
    Qualifications
    • High School Diploma or GED
    • Driver's license (bonus)
    • One-year prior professional experience preferred
    • CHHA - NJ preferred (we can work with you to start a class to receive the license)
    Compensation: $ $200.00 per day

Her sister, Eleanor, is multiply impaired and for her parents, finding reliable, skilled and professional caregivers was not an easy task.Establishing Evolving Care was a natural outcome of Stephanie's work and life experience, which includes a solid foundation of assessing seniors and placing caregivers based on their needs. Prior to that she worked in the fashion industry for a large New York corporation and with a privately owned New Jersey business. Stephanie has a strong liberal arts education from Lafayette College.
