    SUMMARY

    Acts as the primary point of coordination for eligible individuals. SSAs assess the needs and wants of individuals on caseload, develop the Individual Service Plan (ISP), and authorize and monitor the services to ensure individuals are living a good life.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    75%

    • Utilizing Person-Centered Planning, assessment of wants and needs, and assessment of risks, the SSA will learn what is important to and for the person. In collaboration with families, guardians, waiver service providers, and other community members or agencies, the SSA will coordinate services and supports for the individual served;
    • Provide follow-up discussions via phone, email, video, in-person or other contact to aid in the development of outcomes that ensure health and welfare to help the person live their best life;
    • Ensures that services and supports authorized through the ISP process are within an individual's assigned budget;
    • When deemed necessary, develops, monitors implementation, and completes ongoing reviews of Restrictive Measures in accordance with State Rules and Regulations as well as Board Policy;
    • Follows up with service providers to assure services are being provided per the ISP;
    • Provides continuous monitoring of authorized services and reviews the individuals' progress per the proposed or intended outcome of the ISP. Communicates outcomes with the individual's team and provides follow up on areas for improvement;
    • Assures that Level of Care Assessment, and other assigned assessments as identified by County and State rule requirements, are completed and/or coordinated, as applicable;
    • Assists individuals to receive services in the least restrictive manner possible;
    • Assures individuals are connected with appropriate community resources to meet their service and support needs;
    • Educates individuals, and assures they have free choice of provider, as applicable;
    • Completes daily Targeted Case Management (TCM) notes to document all services delivered, TCM notes are subject to subpoena, billable TCM notes are billed to the individual's Medicaid;
    • Adheres to work assignment timelines, including TCM and ISP;
    • Provides excellent customer service in order to support the Board's vision and mission.

    • Follows all State and Federal rules as well as Board policies and procedures;
    • Assures all documents are completed for HCBS Waiver Recipients;
    • Provides crisis assistance to ensure that needed services occur in a critical or unexpected situation (court appearances, arranging for respite, addressing behavioral or medical emergencies, on-call responsibilities, etc.);
    • Assures Medicaid Eligibility is maintained for individuals on caseload;
    • Completes accurate documentation in keeping with requirements of the County Board and any applicable Medicaid requirements;
    • Reports Major Unusual Incidents to include alleged abuse/neglect in accordance with HB 403, Ohio Revised Code 5123, Administrative Rule 5123:2-17-02, and County Board Policies and Procedures.

    • Assists the individual with due process when necessary;
    • Adheres to a flexible work schedule designed to meet individual needs;
    • Trains service providers on ISPs, and verifies implementation of services. Advocates for technical assistance and training to support providers, which may include collaboration with other internal departments, such as Provider Compliance.

    An integral part of the job function is ensuring that services, supports and oversight of such supports occur with the focus on the individual receiving services. Person-Centered Planning is the foundation for all services, supports, monitoring, supervision and coordination for the individuals served. Some positions within the SSA department are not assigned a caseload and complete specific functions as noted above (75%).

    QUALIFICATIONS

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have a valid Ohio Driver's License, an acceptable annual BMV Check, evidence of a good driving record, and proof of insurance.

    EDUCATION AND EXPERIENCE

    Bachelor's Degree in related field. Experience with individuals who have developmental disabilities. Eligible for DODD Certification.
