Job summary
Work with youth and their families at home, at an office setting, in the community, at school and/or at work to pull the youth and family team together to facilitate development of the wraparound plan,
convene regular team meetings, including the youth, their family and their supports,
both professional and natural to monitor implementation of the plan
and make changes to the plan as needed to promote success for
the youth and their family within their community.
