LPN - LICENSED PRACTICAL NURSE - Weekend on Call Friday 5pm-Monday 9am Affinity Hospice is actively looking for a LPN in the Athens area who is passionate about giving exceptional care to our hospice patients.
Responsibilities
LPN Job Summary
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse.
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
LPN Job FunctionsProvides direct patient care within the established Plan of Care and as defined in the State Nurse Practice Act.
Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers. Assists team members as needed to ensure that this level of service is consistently provided.
Implements planed interventions to meet the physical, psychological, social and environmental needs of the patient.
Evaluates the patient's responses to the approved plan of treatment.
Provide accurate and timely documentation of patient services to reflect the plan of care.
Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Participates in coordination of hospice services, appropriately reporting any identified needs to the interdisciplinary group.
Participates in Continuous Care as requested.
Participates in clinical on-call program.
Attends Interdisciplinary Group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement.
Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.
Performs other duties as assigned by the registered nurse.

LPN Position Qualifications
Continuing Education Requirements:
Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor.
This position requires a minimum of 8 education hours.In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities.
All personnel must attend mandatory educational programs.
