A well-established managed care network is currently seeking Physical Therapists who will be participating in the development of patient care plans by completing assessments and implementing treatment programs for older adult and geriatric patients.

*The base pay for this position is $45 to $50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer. This is a temp-to-perm role; you can gain access to company benefits, paid-time, off, and more**Responsibilities:

• Plan and administer treatment programs for patients to restore function and prevent disability
• Complete assessments, progress notes, and discharge summaries
• Perform patient evaluations and treatments
• Aide in the development of care plan with team members
• Participate in care plan meetings in accordance with state and federal regulations
• Maintain updated documents, assessments, and progress notes

*Requirements:

• Master's degree of higher in Physical Therapy
• NY State Physical Therapy License
• BLS Certification
• One year of experience as a Physical Therapist and/or experience with Geriatric population preferred
