    Hartford HealthCare is seeking a full-time, experienced Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly And Care at Home) Program serving our geriatric patients in the greater Fairfield area.

    Homecare based in the greater Fairfield, CT area

    Geriatric Medicine in the home setting, working within a multi-disciplinary care team


    Schedule:
    Full-time, weekdays

    In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

    Unrestricted State of Connecticut Medical License (or eligible)

    Training:
    APRN (Family, Adult/Geri Nurse Practitioner) or PA

    Experience:
    At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA)
