    This key role is client focused and involves consultation and collaboration with clients, participants, families, medical professionals, and other community-based supports to develop and facilitate high-quality in-home care.

    This role is responsible for attending clients' homes and building care and support plans to meet the clients needs and preferences.

    This role works closely with the clinical team to navigate exceptional client outcomes, conduct appropriate assessments, and identify client individualised care requirements.

    The ideal candidate will be able to demonstrate the following.
    Experience in a similar role for more than 12 months
    Excellent interpersonal and communication skills
    Be client and solutions focused.
    Ability to work in a team collaboratively and work autonomously without direct supervision.
    Have a clear understanding of the aged care and disability industries.
    Ability to manage complex client requests and feedback.
    Be approachable, friendly, and empathetic.
    Experience in developing care plans to meet client needs and preferences.
    Demonstrate an understanding of government funded programs.
    Ability to budget client funding to support care plan requirements.
    Computer literate and have the ability to learn new programs quickly.
    Be an out of the box thinker.
    A successful candidate must be able to supply.
    Clean national police clearance
    Current first aid and CPR certificate
    Reliable vehicle
