    As a Registered Nurse Case Manager (RNCM) at our organization, you will:

    • Conduct in-person patient interviews and thorough physical assessments
    • Supervise the execution and continuous evaluation of patient care plans by coordinating with home health aides, LPNs, RNs, and other caregivers
    • Communicate patient status and collaborate with relevant providers to ensure evolving care needs are met
    • Educate patients and families on proper home health care practices including wound care, IV administration, and medication management
    • Focus on reducing readmissions through preventive care promotion and maintaining care continuity

    To be eligible as a Registered Nurse Case Manager (RNCM) with us, you must have:

    • Active, unrestricted Registered Nurse (RN) license in the state of operation
    • Current CPR/AED/BLS/First Aid certifications
    • Reliable transportation for commuting to care sites or work locations with willingness to travel
    • Minimum of one year of professional experience as an RN in home health or a similar environment; prior experience in case management/review is advantageous
    • Preferable experience with OASIS assessment and practical trach/ventilator knowledge (not mandatory)
