    Job Summary

    The Physical Therapist, employed or contracted through the organization, is responsible for delivering high-quality physical therapy services to patients in accordance with individualized care plans and organizational standards.

    The role emphasizes patient comfort, safety, and functional improvement while ensuring compliance with all clinical protocols and regulatory requirements outlined in the Service Employment Agreement.

    Key Responsibilities
    Patient Care and Assessment

    Provide physical therapy services as per a written, individualized plan of care.

    Assess and evaluate patients' therapeutic, rehabilitative, and functional status, including home environment and safety.

    Evaluate muscle strength, mobility, gait, range of motion (ROM), and transfer capabilities, prioritizing comfort and functional maintenance.

    Direct, monitor, and adjust therapy treatment plans as necessary.

    Instruct patients and caregivers in the correct use and maintenance of therapeutic and assistive devices.

    Identify and prioritize physical therapy needs based on patient condition and goals.

    Report any changes in patient condition or reaction to treatment promptly to the physician and clinical supervisor.

    Program Development and Implementation

    Initiate and implement physical therapy programs as part of the patient's total plan of care.

    Educate family members, caregivers, and other healthcare personnel on therapy goals and techniques for ongoing support.

    Participate in interdisciplinary case conferences to align care objectives and progress.

    Documentation and Reporting

    Prepare and submit accurate, timely clinical documentation and progress summaries, including measurable outcomes and goal attainment.

    Participate in discharge planning and provide a final clinical summary as per organizational policy.

    Patient and Family Education

    Provide patient education on safe mobility practices and, when applicable, the use of prosthetic or assistive devices.

    Offer consultation and guidance to families and caregivers to support continuity of care.

    Collaboration and Training

    Collaborate with the interdisciplinary care team to ensure integrated, holistic care delivery.

    Conduct or contribute to in-service education programs for nursing and allied healthcare staff, as required.

    Qualifications and Requirements
    Bachelor's or Master's Degree in Physical Therapy from an accredited institution.

    Current and valid state license to practice as a Physical Therapist.

    Strong interpersonal and communication skills with a compassionate, patient-centered approach.
