- Provide direct nursing care in alternative care environments
- Perform lab draws, IV insertion, and medication administration
- Engage patients in their healthcare journey, offering tailored education
- Establish trusting relationships with each patient
- Implement multidisciplinary care plans for Advanced Care program services
- Collaborate with the Advanced Care leadership team to enhance care delivery
- Coordinate day-to-day clinical activities for patients
- Ensure timely ordering and initiation of necessary services
- Act as the main point of contact for navigated patients
- Participate in patient education and engagement efforts
- Monitor patient progress and track adherence to the care plan
- Assist patients with referrals to community resources
- Contribute to the creation of Advanced Care materials
- Perform assigned job-related duties
- State of Texas Certification/Licensure as a Registered Nurse
- Minimum of 2 years of acute care nursing experience
- BLS and ACLS Certification
- BSN preferred
