As an Advanced Care RN, you play a crucial role in delivering personalized care to patients at home, offering an alternative to hospitalization. We are looking for someone with top-notch clinical, communication, and leadership skills to join our innovative team. Your main responsibilities include providing direct, holistic care to patients and their families in a non-traditional setting. Building strong connections with patients and involving them in the care process are essential for success. In this role, you have the opportunity to contribute to developing a groundbreaking care model, reducing hospital admissions, improving patient outcomes, and enhancing the overall patient and provider experience.

What You'll Do:

• Provide direct nursing care in alternative care environments
• Perform lab draws, IV insertion, and medication administration
• Engage patients in their healthcare journey, offering tailored education
• Establish trusting relationships with each patient
• Implement multidisciplinary care plans for Advanced Care program services
• Collaborate with the Advanced Care leadership team to enhance care delivery
• Coordinate day-to-day clinical activities for patients
• Ensure timely ordering and initiation of necessary services
• Act as the main point of contact for navigated patients
• Participate in patient education and engagement efforts
• Monitor patient progress and track adherence to the care plan
• Assist patients with referrals to community resources
• Contribute to the creation of Advanced Care materials
• Perform assigned job-related duties

What You Need:

• State of Texas Certification/Licensure as a Registered Nurse
• Minimum of 2 years of acute care nursing experience
• BLS and ACLS Certification
• BSN preferred

Physical Requirements:

• Lift & carry up to 50 pounds of equipment/supplies

• Move comfortably in confined spaces
• Maintain balance in different positions
• Stand for extended periods
• Perform repetitive movements

Other Requirements:

• Valid driver's license with clean record
• Complete required drivers training
