    Description


    Position SummaryThe Care Coordinator (CC) is an integral part of the inter-disciplinary Care Management team, and will assist in facilitating patient continuity of care for GLINs highest-risk patients.

    Under the clinical direction and oversight of the Nurse and Social Work Care Managers, the Care Coordinator will assist at-risk patients in various ways with the goal of improving overall patient health.

    To address patient needs and expedite appropriate, cost effective care, the Care Coordinator will assist with telephonic outreaches to patients to engage and enroll in GLIN Care Management programs, and will support the Care Management team with needed provider and community agency collaboration.

    The Care Coordinator will also be responsible for administrative/clerical tasking such as preparing patient Electronic Medical Records (EMRs), requesting/uploading/downloading patient health information, faxing and mailing letters, and answering inbound calls to the Care Management (CM) queue.

    Duties and Responsibilities (including but not limited to)

    Address patient obstacles to careFacilitate engagement between PCP and patientDaily collaboration with Care Management TeamWork as patient advocatePerforms additional duties as assignedAnswer inbound calls to Care Management queuePlace outbound and/or engagement calls as neededPrepare patient charts for potential outreachEstablish patient eligibility for CM programmingUtilize multiple systems to gather informationDocument all communications appropriately in EMRQualifications or Education, Training and ExperienceHigh school or GED diplomaTwo (2)+ years of work experience in one or more of the following settings:

    Acute Care, Ambulatory, Specialty, Physician Office, Care Management, Home Health Care or relatedMust have ability to travel to and from office for required training/ meetingsKnowledge and Skills/Expected CompetenciesMultitasking and prioritization working in a high-volume environmentUnderstanding and implementation of HIPAA and Confidentiality requirementsExcellent interpersonal and organizational skillsCompassionate and patient centric approachAdaptable to new technologies and softwareProficiency (including documenting in) in EMR system(s), Outlook, basic Excel*This is a hybrid-remote position and candidates must reside in WNY to be considered.

    We offer an outstanding benefits package including health, dental, 401K, vacation, and PTO, as well as a great working environmentPay range $18.00 to $24.00The referenced pay range represents the minimum and maximum compensation for this job.

    Individual annual salaries/hourly rates will be set within jobs compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual, and internal equity considerations.
