Job summary
The Home Health Aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs.
We are seeking compassionate individuals who want to make a difference in someone's life by providing excellent patient-centered care.
Duties & Responsibilities
- Providing personal care including: Baths Back rubs Oral hygiene Shampoos Changing bed linen Assisting patients with dressing and undressing Nail & Skin care to prevent breakdown Assisting the patient with toileting activities Keeping patient's living area clean and orderly, as appropriate Planning and preparing nutritious meals. Assisting in feeding the patient, if necessary Taking and recording oral, rectal and axillary temperatures pulse respiration blood pressure when ordered Demonstrating empathy for the needs of ill injured frail impaired Performing range of motion other simple procedures assisting ambulation exercise according to plan of care Doing laundry meeting safety needs using equipment safely properly Reporting on condition changes assigned nurse Attending 12 clock hours annual training Adhering documentation standards professional conduct Attend Case Conference meetings discuss involved patients above statements representative duties performed employee job description may requested perform related tasks stated description.+
