Conduct medical screenings on all new residents to the program within the time parameters of the programs intake process and procedures.
Deliver medication management (individual/group) as required by program or resident including assistance with prescription refills, medication changes, prior authorizations, pharmacy issues, and medication reconciliation prior to a pending a hospital discharge.
Maintain updated progress notes of clinical and medical interventions.<
