Responsibilities

About the Position:

Matrix providers review a member's health, medical history, and social environment to deliver comprehensive assessments that lead to improved care quality.

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN-Flexible-Nurse Practitioner

Location: Fairmont, West Virginia

Hours: Flexible scheduling including days, evenings, nights, weekends

PAY: $90.00 per completed assessment, flexible scheduling

Benefits: Limited voluntary benefits for PRN providers

Mileage reimbursement for 20+ completed assessments/month, No show payment for 10+ booked assessments/month

Matrix Provider – What to Expect:

• Conduct Adult/Geriatric Assessments, medical history reviews, diagnosis and treatment planning
• Work collaboratively with medical professionals and caregivers
• Collaborate with Primary Care Physicians for patient education
• Services provided in various settings including Home Visits, Skilled Facility Visits, and Telehealth

THIS IS NOT A FULL TIME JOB – It is PRN/COVERAGE with no guaranteed hours

Qualifications

Must Haves:

• Master's Degree or equivalent experience, NP licensure
• Current RN and NP licensure, board certification in a relevant specialty
• Valid BLS, ACLS or CPR certification
• Minimum 1 year experience as a Nurse Practitioner
• Strong computer skills, excellent communication abilities
• Flexibility and adaptability to change

Travel Requirements:

• Ability to travel with valid driver's license and car insurance
• Travel may be necessary
