    Spiritual Care Counselor

    Care Partners, a nonprofit Hospice & Palliative Care provider, is recruiting for a per diem Spiritual Care Counselor. This position will provide spiritual care services to patients and families/caregivers, either directly or through coordination of care with area clergy, spiritual care volunteers or other staff spiritual care counselors. This role will be part of a multi-disciplinary clinical care team and plays a key role in delivering spiritual and emotional support to patients and families and enhancing sensitivity and understanding for staff of spiritual concerns in our settings. Care Partners is a healthcare organization committed to honoring choice and dignity for patients at end-of-life while providing comprehensive patient and family care.

    Your Responsibilities:

    • Complete initial and ongoing Spiritual Assessment of patient/family/caregivers and assure appropriate spiritual and pastoral services are provided in a timely manner.
    • Develop a plan of care addressing religious and spiritual care needs in collaboration with patient, family and other clergy, spiritual car counselors and volunteers implementing appropriate interventions or referrals.
    • Provide direct spiritual care and emotional support to patients/family/caregivers, revising frequency of visits and resources utilized as indicated through care conferences and IDG.
    • Complete documentation in patient records noting visits, contacts, plans, action, goals and changes using clearly stated notations as they occur.
    • Provide appropriate support at time of death to patient/family/caregivers and other IDG members.
    • Provide on-call support to patient and family when appropriate and in collaboration with colleagues.
    • Ensure that the religious beliefs, practices, and values of those served are honored and respected within the total healing process.
    • Complete annual mandatory training and participate in professional development opportunities.
    • Assist in conducting bereavement and memorial services for staff, patient families and community partners as needed.
    • Maintain confidentiality and compliance with HIPAA guidelines and foster an environment that encourages professionalism and discourages disrespectful behavior.

    Qualifications:

    • Must be a graduate of an accredited seminary or school of theology and hold a Masters' degree in Divinity or Theology or equivalent.
    • Must have a minimum of two years of experience as a spiritual care provider in a clinical setting and five years total between clinical and experience in other worship settings.
    • Experience in work with hospice patients and families, including knowledge of and commitment to hospice philosophy of care is preferred.
    • One unit of Clinical Pastoral Education (CPE) from an accredited program is preferred.
    • Board Certification through recognized certifying body or membership in a professional chaplain association is preferred.
    • Excellent oral and written communication skills and the ability to demonstrate understanding and compassion when working with challenging patient and family situations and community partners is required.
    • Must communicate effectively with children, adolescent, adult and geriatric patients and families based on principles of growth and development across the lifespan and work with high standards of ethics.
    • Must be reliable and dependable in organizing and prioritizing work, completing work on time, following through to complete tasks, and be available for scheduled hours.

    Schedule: This is a per diem position, with normal work hours falling between 8am – 5pm, Monday through Friday. On call, weekend or holiday coverage may be required. Schedule to be determined at time of hire.
