- Permanent address must be at least 60 miles from the facility
- Cannot have worked as permanent staff at any LifePoint facility in the past 12 months
- Cannot be employed elsewhere full time during the assignment
- Active VA or compact RN license required
- 1-2 years of recent Operating Room RN experience required
- Certifications must be from recognized providers; RQI certifications are not accepted
- Must provide Social Security number, date of birth, and COVID vaccination documentation for consideration
