This includes performing reassessments and updating treatment plans based on patient response, documenting evaluations, treatments, and outcomes in the medical record, collaborating with care teams to coordinate therapy plans, and discharging recommendations. The ideal candidate should have excellent communication skills both verbally and written form as well as ability work independently but also part of an inter-professional healthcare team.
