    Description

    Overview

    Duties include, but are not limited to:

  • May perform initial and interim histories and physical examinations on both inpatients and outpatients assigned to his area. The initial H&P will be reviewed by the supervising physician, or the designated alternate, and countersigned. Regular review of care rendered to stable patients will be accomplished. Initial Progress Note will be reviewed by supervising physician or designated alternate for concurrence on all patients.
  • May screen patients to determine the need for hospital admission and may so admit patients when indicated and inform the receiving physician of the admission as soon as practicable. The physician assistant is prohibited from rejecting patients for admission or discharging patients from care without the concurrence of a physician.
  • May order routine laboratory, radiologic and diagnostic tests on patients assigned to his area and provide initial interpretations of results.
  • May order EKGs and provide initial interpretation of such.
  • May order x-rays as needed on assigned patients.
  • May order pulmonary function testing on assigned patients.
  • May perform venipuncture and arterial punctures to obtain specimens.
  • May initiate/expedite requests for consultations to other services. Non-VA consults may be initiated after discussion with supervising physician and require co-signature.
  • May order and/or give oxygen as necessary.
  • May order diets and nutritional supplements as necessary.
  • Conducts compensation and pension exams utilizing Data Based Questionnaire (DBQ) format.
  • May start intravenous (IV) solutions and administer IV medications as necessary.
  • May order/insert/remove nasogastric catheters and suction, lavage, gravage as necessary.
  • May order/insert/remove urinary catheters as required for the patient.
  • May re-insert GT tubes as necessary.
  • May irrigate ears and remove impacted cerumen.
  • May request home devices, equipment and supplies.
  • May dictate discharge summaries on assigned patients. A discharge note will always be done by the physician or alternate and co-signed.
  • May provide patient and family counseling within areas of expertise.
  • May apply accepted preventive health care measures to assigned patients.
  • May initiate in-house transfers to other units.
  • May perform pelvic examinations and/or Pap smears.
  • May obtain consent for any procedure the PA is performing.
  • Prescriptive Authority has been granted to the Medical Service Physician Assistant in accordance with VHA Directive 1063, Utilization of Physicians Assistants (PA).
  • The prescribing practices of the PA are included in the medication use evaluation process. Both Clinical Practice and prescribing practices will be monitored quarterly.
