    Adecco Medical & Science has an outstanding opportunity for a
    Hybrid RN based in
    Manhattan, New York . This is a
    Full-Time position and allowing you to work alternating in home and virtual setting.


    Pay Rate:
    $55-$58/hr.


    Schedules:
    Schedules vary.

    Monday-Friday 8:00AM-6:00PM

    Sat & Sunday 10:00AM-6:00PM


    Hourly wage may depend upon experience, education, geographic location and other factors.

    Qualifications

    • Bachelor's Degree from an accredited nursing program, preferred.
    • New York State RN License, required.
    • Work experience with UAS-NY or Oasis Community Assessment tool, required.
    • 2+ years' experience working inhome setting within the last 10 years, required.
    • Work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities.
    • Computer literacy: ability to navigate new and emerging technology; ability to perform telehealth assessment; highly developed written and oral communication skills; strong interpersonal skills; ability to work in a stressful and changing environment; and proven ability to work with community groups.
    • Knowledge of longterm care services and programs
    • Ability to travel throughout the assigned region, as assigned, including travel to individuals' homes.

    Responsibilities

    • The incumbent will review the completed Community Health Assessment tool in the NYS Uniform Assessment, to assist the Physician in the completion of a Physicians order.
    • The incumbent may follow up with individuals who have received an assessment through facetoface communications.
    • Responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.
    • Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
    • Work collaboratively to monitor and address clinical quality and gaps in care.
    • Provide clinical and educational assistance to support project initiatives.
    • Demonstrate communication skills necessary to interpret agespecific responses.

    Pay Details: $55.00 to $58.00 per hour
