Job Title: Direct Support Specialist (DSS) -Independent Contractor

Summary: Carry out the mission to provide quality care, life enrichment, indefinite possibilities, and a family atmosphere to those diagnosed with disabilities.

Independent Contractor Responsibilities:

Must have all required training before starting the position, including driver's license, insurance, CPR, first aid, medication cer1, 8 hours of DODD training certificates, diploma, driving abstract, background check, and proof of at least five years of work.

• Understand that this is a temporary contracted 1099 position.
• Must be willing to work at least 35 hours at three different locations.
• Must be willing to transport.
• Must sign a 6-month contract.
• Must provide proof of all training and certification before working and renewals while the contract is still active. All documents must be active and up to date during employment status.
• Must be willing to work weekend shifts.
• Must be willing to do some on-call shifts.
• Must have an account for direct deposit only.
• Must attend all mandatory training.
• Must follow all of the guidelines a DSP is required to follow.
• Must be able to keep contract information confidential. Violations will be grounds for termination of the contract.
• Must complete company overview.
• Must complete all in-home training and documentation.
• Must sign off on all rules, regulations, policies, and procedures.
• Understand that payment is based on documentation requirements.
• Ensures that each individual's needs are met, including personal care, mealtime, medication administration/assistance, equipment adaptations, daily household chores and errands, vocational opportunities, and transportation.
• Assist in developing a positive living and work environment by being proactive and preventing behaviors.
• Supports each individual's physical, social, emotional, and intellectual growth.

Communication: Encourages and maintains good communication with co-workers, families, school, work settings, neighbors, and community contacts.

Represents C&J Tender Heart R.S. positively and professionally.

Recordkeeping: Maintains complete and accurate records regarding individuals' progress, health problems, services, program implementation, incident reports, financial transactions, and household operations. Compiles and submits information and reports specific to the employee's department.

• Must be able to handle money and financial transactions and records.

Housekeeping: Perform housekeeping and maintenance tasks as required

• Ensures sufficient food and household supplies are kept on hand and reports other needs as necessary.
• Effectively and respectfully communicates with individuals within the agency and those in the community.

Staff Development and Training: Attends and participates in staff meetings and in-service training to continue developing and sharing job-related skills. May assist in the training of other staff.

Scheduling: A Direct Support Specialist DSS will be scheduled flexibly between work sites to best meet the needs of the individuals receiving services. Staff will be trained and required to work in any assigned area.

• Medical Appointment DSS Additional Responsibilities:
• Schedules, transports, and attends individual medical appointments, tests, and/or procedures. Schedules follow-ups as needed.
• Identifies and organizes items needed for approaching appointments, including documentation.

Follows all C&J Tender Heart R.S. transportation policies and procedures, whether in a C&J Tender Heart vehicle or in the employee's vehicle.

• Reports any maintenance issues or concerns.
• Report all accidents and traffic violations.
