    On-Call Requirements:
    Nights & Weekends

    License to Submit:
    Active Illinois RN license required (or proof of application initiated)

    Unit Overview

    Department:
    Home Care – Hospice (Rockford)

    Patient Population:

    End-of-life care including:
    All types of cancer
    End-stage COPD, CHF, CKD
    Alzheimer's Disease
    Parkinson's Disease

    ALS
    Liver disease
    Other terminal conditions

    Daily Caseload:
    ~5 visits per day

    ‍ Required Experience & Skills

    Minimum

    1 year hospice experience
    Strong pain & symptom management skills
    Experience in pronouncing deaths
    Knowledge of common end-of-life medications
    Experience with central lines
    Epic experience required
    Equipment Experience

    Trilogy ventilator
    BiPAP

    CPAP
    Scheduling & Call Requirements

    8-hour shifts (0800–1630)

    Weekend backup on-call:
    0800–2000 Saturday & Sunday
    Every 6 weeks

    Minimum of 2 weekend backup shifts

    Weekday on-call:
    2300–0800 as needed (approximately 1x per week)

    1 holiday required
    Dress Code

    Clinician provides own scrubs
    Any solid color, clean and professional
