This position will be the Acting Program Manager/Charge RN of the Adult Day Care Center while Adult Day Care Manager is out on personal leave.
The Adult Day Care Program Manager Sub supervises the LPN, CNAs and volunteers while at the Adult Day Care Center.
Essential FunctionsConducts client intakes.
Assesses clients and formulates individual care plans that comply with the Department of Elder Affairs (DOEA) and Agency for Health Care Administration (AHCA) requirements.
Maintains client confidentiality and handles client files in compliance with HIPPA requirements. Maintains client charts to include all necessary documentation.Maintains and submits program statistical reports as required. Re-evaluates the needs of each client and document in their care plan on a quarterly basis. Monitors client vital signs and reports any changes to responsible party or physician as necessary. Counsel's participants and caregivers regarding topics related to health and well being. Develops and encourages program activities that promote client mental and physical stimulation, self-esteem and social skills. Creates a safe, comfortable and welcoming atmosphere. Supervises staff and volunteers to assure appropriate interaction and activities with clients. Provides and/or participates in necessary training. Attends staff meetings, conferences, workshops, and seminars as required. Maintains State required CEU's for RN licensure and CPR certification. Performs related work as required. During emergencies, all employees are considered essential personnel and may be required to perform alternate duties.

Minimum Qualifications
Associate or Bachelor's degree in Nursing from an accredited College or University. Possess a current State of Florida RN license in good standing. Possess a current CPR/AED certification.
Must complete 24 continuing education units every two years to include State of Florida course requirements to maintain RN licensure.
Minimum 4-years nursing experience, 2-years in a supervisory position working with Alzheimer's and dementia patients.Flagler County is a drug free workplace, some positions may require a pre-employment drug screen.

Knowledge, Skills and AbilitiesExcellent interpersonal and communications skills. Strong command of English both oral and written. Must have the ability to work well with others.
Must have the ability to manage multiple priorities and possess good organizational skills. Must possess computer skills.

Physical Demand:
Heavy
Lifting, stooping, bending, pushing and assisting clients to stand. Full use of upper and lower extremities and back muscles. Use of proper body mechanics to avoid back injury. Good hearing and eyesight with or without corrective lenses.
