Job Summary
The Case Manager I develops and implements a person-centered program for assigned clients to achieve individual goals.,Responsibilities
Provide direction supervision and case management for assigned clients.,Complete an initial person-centered plan and assessment develop an individual program plan document progress for the client.,Act as Chair for annual staffing meetings of unfunded clients function as part of Interdisciplinary Team to plan and develop person-centered program.,
- Plan ancillary activities for clients including special staffing initiate appropriate referrals.
- Encourage independence by assisting in problem-solving using accepted innovation rehabilitation techniques.
- Teach proper work habits interaction skills self-help skills through role modeling intervention.
