Description
We are located just South of Boston, 45 minutesfrom Providence RI and are searching for a Hospitalist Physician Assistant or Nurse Practitioner towork in a collaborative role with theHospitalist physician in providing care to a diverse patient population in aninpatient medical service.
Primary Responsibilities:
· Performsinitial history and physical examinations on patients being admitted to thehospitalist service
· Documents viawritten and dictated notes after collaboration with staff hospitalistphysician.
· Dailyrounding visits on assigned patients, discusses findings and with staffhospitalist physician and collaborates with said physician to determine,document, and implement treatment plan.
· Communicateswith consultant services regarding patient care and needs.
· Communicates withprimary care physicians regarding patient's admission and discharge plans.
· Facilitatescommunication between hospitalist service and patients.
· Communicateswith allied health professionals to include (but not limited to) casemanagement, social work, physical therapy, nutrition, and wound management toensure complete patient care and appropriate and timely discharge planning.
· Coordinates dischargeplanning including completing appropriate forms and documentation.
· Performsother duties as required or assigned.
Education: Graduate of an approved Physician Assistantprogram or Nurse Practitioner Program; Bachelor's degree required; Master's Degree preferred. Must havecurrent valid Massachusetts license and be prescriptive eligible.
