Job summary
The Scheduler plays a vital role in supporting timely, compassionate Home Health care across Southeast Michigan. In this position, you will coordinate patient schedules with a focus on clinician continuity, ensuring that each individual receives care from the most appropriately skilled team members.Responsibilities
- Schedule Home Health visits by matching client needs with clinicians who have the appropriate competencies.
- Follow established policies and procedures to ensure scheduling accuracy, efficiency, and continuity of care.
- Develop and maintain daily schedules for all clinical disciplines, striving to limit clinician variation within each patient episode.
