    **Sign-On Bonus: $1500 after six months in role**

    Juvenile Justice Mental Health Liaison

    Location: Rockford, IL

    Position Summary

    The Juvenile Justice Mental Health Liaison utilizes the mental health screening tools and comprehensive assessment to assess the client's needs for mental health treatment and then works collaboratively with the individual youth, their family, the youth's integral health home, and referring individuals from either the Illinois Juvenile Court System, probation, school, or community, to link the youth and family with any necessary community-based mental health and social services. The position provides timely completion of mental health evaluation and assessment data to help determine appropriate services needed.

    Responsibilities

    The Juvenile Justice Mental Health Liaison will perform duties including, but not limited to:

  • Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies.
  • Provides appropriate individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds on behalf of persons served.
  • To strengthen and increase community partnerships with linkages among courts, probation, child protective agencies and detention, schools, and other community-based services.
  • Facilitate the identification, screening, referral, and case monitoring of at-risk youth who are identified as having a mental illness, including post-traumatic stress reactions? or services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
  • Build collaborative relationships with the local Juvenile Court, probation, school, and community to provide training and technical assistance in understanding the potential mental health needs of youth involved in their system.
  • Regularly reviews and modifies youth-based assessment and treatment plans to ensure consistent progress and success on written plan goals.
  • Utilize the MHJJ referral screening form to determine if a mental health assessment is necessary.
  • Track the number of referrals from each referral source and report data to the DHS/DMH on a quarterly basis.
  • Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.
  • Knowledge and experience of psychiatric diagnosis and recognizing signs/symptoms of psychiatric illness and substance-related diagnoses.
  • Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis.
  • Actively participates in the weekly clinical supervision process, team meetings, and professional development activities.
  • Travels daily, to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
  • Completes all required documentation in a timely manner.
  • Completes all required training in a timely manner.
  • Performs other duties as requested.
  • Minimum Qualifications

  • Master's degree in social work, counseling, or human services
  • A valid professional state license (LSW, LPC)
  • 2 years of work experience working with youth and families in a community and a home-based setting is strongly preferred.
  • Experience working in social services, juvenile or probation files with knowledge of the court system.
  • Excellent written and oral communication skills, interpersonal skills, customer service, and organizational skills.
  • Ability to relate therapeutically with customers and display cultural sensitivity and a strong sense of empathy.
  • Capacity to remain objective and professional in all areas of job function.
  • Ability to organize data, plan, provide written and very feedback, and utilize electronic health systems.
  • Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.
  • Ability to manage multiple tasks.
  • Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
  • Driving and Vehicle Requirements

  • Valid driver's license
  • Reliable personal transportation
  • Good driving record
  • Minimum automobile insurance coverage of $100,00/$300,000 bodily injury liability
