    Overview: As a Case Manager at our addiction treatment center, you will play a pivotal role in providing comprehensive support to individuals struggling with substance abuse disorders. You will be responsible for assessing clients' needs, developing personalized treatment plans, coordinating services, and advocating for their well-being throughout their recovery journey. This role requires a compassionate and dedicated individual with strong organizational and interpersonal skills.

    Responsibilities:

    1. Client Assessment: Conduct thorough assessments to evaluate clients' substance abuse history, mental health status, medical conditions, and social support networks.
    2. Treatment Planning: Collaborate with clients to develop individualized treatment plans tailored to their specific needs, goals, and preferences.
    3. Case Coordination: Coordinate and monitor the implementation of treatment plans by liaising with internal and external service providers, including therapists, medical professionals, social workers, and support groups.
    4. Advocacy: Advocate for clients' rights and access to necessary resources, services, and support systems within the community.
    5. Crisis Intervention: Provide immediate assistance and support during crises or emergencies, including overdose incidents or relapse episodes.
    6. Progress Monitoring: Regularly evaluate clients' progress towards treatment goals, adjust plans as needed, and document all interactions and interventions accurately.
    7. Education and Support: Offer education, guidance, and emotional support to clients and their families on addiction, recovery strategies, coping skills, and relapse prevention techniques.
    8. Referrals and Aftercare: Assist clients in transitioning to aftercare services, such as sober living facilities, outpatient programs, vocational training, or ongoing therapy, to support their long-term recovery.
    9. Collaboration: Collaborate closely with other members of the treatment team, including therapists, counselors, medical staff, and administrative personnel, to ensure coordinated and effective care delivery.
    10. Compliance: Ensure compliance with legal and ethical standards, confidentiality regulations, and organizational policies and procedures at all times.

    Qualifications:

    1. Bachelor's degree in social work, psychology, counseling, or a related field. Master's degree preferred.
    2. Certified Alcohol and Drug Counselor (CADC) or similar certification preferred.
    3. Previous experience working in addiction treatment or mental health settings, preferably as a case manager or counselor.
    4. Strong understanding of addiction, recovery principles, and evidence-based treatment modalities.
    5. Excellent communication, interpersonal, and problem-solving skills.
    6. Ability to work effectively both independently and as part of a multidisciplinary team.
    7. Empathy, patience, and cultural sensitivity when working with clients from diverse backgrounds.
    8. Proficiency in maintaining accurate documentation and utilizing electronic health records (EHR) systems.
