SummaryThe Care Concierge (CC) role plays a crucial part in achieving Curana Health's mission to improve the health, happiness, and dignity of our senior living residents.

The Care Concierge will often have the single most interactions with our patients and their family members in the community.

Patients should know their CCs on a first name basis and create a tremendous relationship with them. CCs perform various administrative duties that empower their care team to operate at the top of their license(s). The patient/family experience should be directly correlated with the strength and success of this role. We strive for exceptional care, experience, and communication.
Job Duties and ExpectationsFacility EngagementCoordinate and attend facility and Curana Health educational/social events.

Act as community liaison between key stakeholders (nursing, sales, wellness, and leadership)Be attentive to the needs of the facility and communicate back to the team.

Patient EngagementCoordinate care between (specialists, therapy, home health, and other ancillary services)Assist with front desk duties including appointment management, scheduling, and other related duties.

Monthly outreach with residents/family to assess needs and coordinate care consistent with provider's plan of care.
Clinic Setup and DesignAssist with clinic set up and design within Curana Health specs.
Ensure proper collateral and supplies are ordered in a timely manner.
Clinical FunctionsGreet each patient, family member, and staff member with a smile.

Curana will train you to be able to take vital signs, room patients, and help get them ready for their visit.

Serve as backup to Medical Assistant in Curana Health Clinic.

QualificationsHigh School Diploma or equivalentExperience in the senior living setting strongly preferredOutgoing individual who loves interacting with peopleOrganized and effective communicatorConcierge or hospitality experience a plusExperience as a medical assistant, activities coordinator, social worker, or LPN a plusProficient in Microsoft OfficePositive attitude and strong interpersonal skillsPassionate about helping othersHighly organizedWillingness to learn and be a part of something greatCurana Health is dedicated to the principles of Equal Employment Opportunity.
