    The client will not be providing equipment.

    It is expected, per the program, that the firm will provide the necessary and properly secured equipment needed to perform the job.

    As a Quality Assurance Analyst, you will collaborate with client and NYSDOH team members to assist with the Health Home Re-designation process.

    Your day-to-day role as a consultant will be providing quality assurance and quality improvement support including but not limited to policy and procedure reviews, patient chart reviews, documenting findings, and developing and reviewing comprehensive re-designation final reports for all health homes across New York State.

    Review and evaluate assigned health home policies to determine compliance based on state and federal policy requirements
    Document review findings utilizing the processes provided.
    Conduct Quality Assurance reviews on completed charts to ensure accuracy of data collected.
    Synthesize and summarize data obtained from the Case Management Agency survey.
    Develop training materials to educate state agency staff on new processes and procedures.

    A minimum of a bachelor's degree in Public Health, Social Work, Healthcare with at least 2 years of related experience is required.

    An equivalent combination of advanced education, training, and experience will be considered.
    Working knowledge of Health Homes Redesignation program, including a good understanding of the redesignation process and requirements.
    Experience with multiple Electronic Health Record applications.
    Ability to work with and relate to staff and demonstrate active listening skills.
    Ability to foster teamwork with all levels of management and staff.
