Job Summary
The Oregon Primary Care Association (OPCA) seeks a Practice Transformation Senior Manager to coordinate and handle day-to-day activities related to process improvement at community health centers in Oregon and SW Washington alignment with the Mountain West AIDS Education and Training Center (AETC) & Oregon's End HIV initiative.The Practice Transformation Senior Manager works directly with Federally Qualified Health Centers (FQHCs) across the state to improve health outcomes along the HIV care continuum and coaches for practice change, with an emphasis on quality improvement initiatives that improve access for people experiencing systemic barriers to care.
This position bridges the gap between best and promising advances in clinical practice, social determinants of health, and systems level interventions needed to support positive health outcomes in Oregon.
Responsibilities
- Explore and evaluate opportunities for program growth to address prevention screening, linkage to care and treatment, viral suppression of HIV in workforce development along the HIV Care Continuum.
- Apply knowledge technical expertise assist FQHCs developing implementing problem-solving approaches complex challenges experienced people living acquiring HIV those risk acquisition.
- Maket facilitate learning opportunities training events communities practice webinars conferences healthcare systems support practice change.
