    Position Summary

    The Salvation Army Duluth Corps provides case management, staffing, and life skills programming to Plover Place, a 24-unit permanent supportive housing program for single adults experiencing homelessness or at risk of homelessness. The Case Manager is responsible for providing a full range of case management services to the residents.

    $21.00/hour, 40 hours/week

    Essential Functions (approximate time spent)

  • Provide long-term strength-based, person-centered holistic case management. (60%)
  • Coordinate services for participants with collaborative agencies. (15%)
  • Assist with program fiscal responsibility including budgeting, grant spending, and billings. (15%)
  • Maintain up-to-date, accurate participant information in casefiles and through database entry. (5%)
  • Engage in internal and external activities/meetings to promote and further The Salvation Army's goals and objectives. (5%)
  • Education and Experience

  • Bachelor's degree in a relevant field. Instead of a degree, education, and experience may be considered.
  • If the degree is in social work, must be licensed by the state board.
  • Experience working in a social service setting conducting long-term case management preferred Must pass background checks, possess a valid driver's license, and meet MVR requirements.
  • Must have at least one year of experience with the target population served.

  • Proficient with PC, Microsoft Office, databases, and standard office equipment.

  • A variety of work including sitting, standing, walking, reaching, bending, typing, and lifting.
  • Professional environment; not eligible for a remote-friendly work environment.
  • Must be able to communicate clearly in English.
  • Standard business hours with occasional evenings and weekends.
  • Must be able to occasionally lift 20 pounds
  • Supervisory Responsibilities

    None.

    Travel/Driving Requirements

    Drive agency vehicles locally to facilitate case management services.
