    POSITION SUMMARY Provides rehabilitative services to home health and hospice patients.

    Carries out skills contributing to treatment, rehabilitation, and safety for home health patients and maintenance, education, and safety for hospice patients and families.

    Plans for coordinated therapy service for individuals and families under her/his care. Appraises individual and family health needs and hazardsexisting or potential. Consults with and refers families to appropriate personnel within the agency or community services. Treatment and preventative procedures are carried out under medical directions.

    The Home Health/Hospice physical therapist works under the supervision of the supervisor/manager in evaluating, planning, implementing, supervising or documenting the care of patients specific to population served with special attention with geriatric population.

    This care will be delivered in accordance with established standards of performance. MINIMUM QUALIFICATIONS AND REQUIREMENTS Licenses and Certifications Required Licensed Physical Therapist in North Dakota. Current driver#s license required and car available to travel to patient#s home. Educational Requirements Experience Requirements Strongly prefer 2 years previous experience as a Physical Therapist. Special Skills or Training Requirements Organize time well and meet job expectations and performance standards. Exhibits responsibility and flexibility in work situations, willing to work irregular and/or less favorable hours. Demonstrates tact, sensitivity, sound judgment, and professional attitude when relating with patients, families and colleagues. Meet the physical and emotional demand of the position. Demonstrate a caring and respectful attitude when providing patient care.
