Job Description - Nurse Case Manager

Companion Hospice

SUMMARY:

The Nurse Case Manager is responsible for providing nursing expertise and ensuring all services needed are identified, outline in the Plan of Care and delivered to hospice patients and families.

May be assigned visits, admissions or RN triage duties.


RESPONSIBILITIES TO INCLUDE:


Develops a detailed Plan of Care for each assigned patient in conjunction with the Hospice Interdisciplinary Team (IDG), patient, family and attending physician.

Assures all services indicated in the Plan of Care are provided.

Participates in IDG as a core team member.

Supervises Home Health Aides/Certified Nurses Aides and Homemaker.


Reports to the attending physician and IDG any significant changes in the patient's condition and requests change of orders and updates the Plan of Care as needed.

Ensures coordination of care across all settings.

Responsible for pain and symptom control.

Instructs patient and family on techniques of patient care.

Reviews with patient and family the Medicare Hospice benefit and all services that are available.

Completes all required documentation.

Participates in in-service training for staff and contracted facilities.

Participates in CQI activities.

Complies with the state and federal regulatory requirements.

Has a responsibility to participate in staff training and education programs.

Participates in on call duties and admission procedures.

Performs other duties as assigned.

POSITION QUALIFICATIONS:
Registered Nurse with current state license.

One year of recent medical, surgical, oncology or home care experience.

Knowledge of pain and symptom control related to terminally ill patients.

Complies with acceptable professional standards and practice.

Demonstrates good communication, negotiation and public relations skills.

Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Reliable transportation, current driver's license and current auto insurance.


PHYSICAL DEMANDS:

The employee is regularly required to sit at desk or in meetings for long periods of time; to stand, walk, climb, stoop, and kneel, for two to five minutes intermittently throughout the day; to use hands and fingers, and handle and feel objects such as patients supplies or office equipment; to drive to and from appointments; to reach with hands and arms; to lift, carry, push and pull 20-25 lbs intermittently; to talk; to hear; and to have clarity of vision at 20 inches or less and at 20 feet or more with the ability to adjust focus in order to read and write reports and to drive safely.


WORK ENVIRONMENT:


The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.


The employee regularly travels to patient homes and community appointments in a variety of weather conditions and will be exposed to the weather while walking to and from the vehicle.

The employee may occasionally be exposed to noise or vibrations while traveling or working with office or medical equipment and will rarely be exposed to electric shock, radiation, toxic waste, fumes, odors, dust, contagious diseases, blood borne pathogens, animals, or pests while performing duties within the community or in patient homes.
