    As an MDS Coordinator, you play a crucial role in equipping your team with the knowledge required to attend to each resident's specific needs. Your role involves developing, monitoring, auditing, and adjusting individualized care plans to uphold the high standard of care at Avantara Evergreen Park. This includes conducting resident assessments and supporting in the discharge procedure. The residents rely on your expertise, abilities, and meticulousness to ensure their well-being and comfort.

    To be eligible for this role, candidates must hold an unencumbered State of Illinois R.N. License, be a graduate of an accredited nursing program, and possess C.P.R. Certification. While prior experience as an MDS Coordinator, one (1) year as an R.N. in a Skilled Nursing Facility, and (1) year as a C.N.A. are advantageous, we are open to investing in the right individual.
