    Job Description

    The Nurse Care Manager (NCM) is responsible for lateral integration of care across all settings in which health care is delivered to his/her assigned patients. Through the assimilation and application of evidence-based information, the NCM designs, implements and evaluates treatment plans by coordinating, delegating and participating in the care provided by the health care team. The NCM is responsible for working with the health care system to promote an environment that will empower his/her assigned patients to manage their illnesses and achieve optimal outcomes. In addition, the NCM will provide and manage care, and assist in the tracking of healthcare outcomes for his/her Health Home enrollees.

    BSN or the equivalent in a combination of an ADN and relevant experience

    Current Missouri License as a Registered Nurse.

    Current BCLS

    Minimum of 3 years of experience working in a healthcare setting

    Must be able to read and speak the English language

    Proficient in Word, Excel, and Power Point

    Preferred Requirements:
