Works as a liaison to grant support to the vulnerable members of the community addressing material and health challenges maintains a caseload of members provides referrals to various support services by conducting assessments and in home visits serves as a role model identifies initiates referrals to social service programs serves as main point of contact for member supports implementation of various care activities
