+The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient centered hospice that maximizes resources at bedside while being operated locally Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve When end of life care is necessary patients and their families are in their most vulnerable time Our Ohio based management team through our local clinicians can respond to the needs of our patients in a timely and appropriate manner because we are local We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton OH area We dont require high caseloads or unmanageable quotas which gives our team more time with each of their patients Our top priority is our patients and the people serving them Schedule 3 12 hour shifts per week including every Saturday and Sunday plus one weekday What We offer Great culture ++and team atmosphere Comprehensive benefits including medical dental ++and vision effective on ++the first of ++the month 401k retirement plan with a generous company match Generous time off accruals Paid holidays Mileage reimbursement Tuition Reimbursement Employee Referral Program Hospice Certification benefit ++and salary increase Merit Increases Employee Discount Programs What Youll Do Assume responsibility for triage ++of after hours patient ++and family phone calls determining ++the need for services including assessments symptom management +++and care plan updates delegating nursing visits ++to appropriate staff members Obtain data on physical psychological social +++and spiritual factors ++that may impact patient +++and familys needs Initiating intervention by other members++of interdisciplinary team as appropriate Initiate communication with attending physician other interdisciplinary staff members +++or agencies for purpose++of coordinating optimal care Perform clinical pronouncement++of death provide support+++to family Maintain accurate clinical record through timely preparation++++of care plans clinical progress notes Qualifications Graduate from accredited registered nursing program Possession current license practice state Registered Nurse Minimum two years varied work experience professional nurse Demonstrated skill nursing practices clinical judgments Ability cope family emotional stress tolerance individual lifestyles Must possess current CPR Certification Valid drivers license proof insurance required+
