Job Summary

The Social Worker (MSW) at Avera plays a vital role in our clinical care team, responsible for planning, organizing, and implementing social work services to patients and families in the clinic setting. This position requires a Master's degree in Social Work and involves assessing patient needs, facilitating plans to promote effective care, and maintaining a current knowledge base of community agencies and resources.

Key Responsibilities

• Assess patient needs and develop individualized care plans
• Facilitate communication between patients, families, and healthcare providers
• Provide direct clinical therapeutic services to patients as part of an overall comprehensive patient care plan

Requirements

• Master's degree in Social Work from an accredited Social Work Program
• Licensed as a Social Worker in the State of South Dakota

Salary Information

The hourly range for this position is $28.75 - $43.50 per hour, dependent on experience.
